About this study

The purpose of this study is a continuation of the Phase I feasibility study where the investigators fully met and exceeded project aims to design, build, and test the usability and acceptability of an avatar-based system, the Virtual-CAMS (V-CAMS, formerly called CAMS-RAS), for use by suicidal ED patients and medical personnel responsible for their care.

Phase II includes a formative phase, during which the investigators will continue conducting usability and acceptability tests of new features, and a summative phase to conduct a clinical trial. SBIR Phase II project aims include: (1) Complete development of V-CAMS, including readiness for electronic health record integration; (2) Cultivate and utilize the advisory board so that V-CAMS is positioned to meet the needs of diverse healthcare systems and successful conduct of the proposed research; (3) Conduct usability/acceptability tests of new features with target end-users (suicidal patients, including those in the ED, medical providers) and key stakeholders (i.e., administrators); and (4) Conduct a randomized controlled trial (RCT; N=90) of suicidal ED patients comparing V-CAMS (n=45) to Care-As-Usual (CAU; n=45). Participants will be assessed at baseline while in the ED, and again at 7, 30, and 90 days. The investigators predict that V-CAMS participants will report a significantly greater decrease in suicidal behaviors and ED/hospital admissions, as well as significantly greater increases in use of behavioral coping skills, self-efficacy in coping with distress, and perceived helpfulness of patients' ED experience.

About this study

The purpose of this study is to evaluate mindfulness intervention in association with breastmilk volume output in mothers who are pumping breastmilk in the NICU. To evaluate associations between mindfulness intervention and levels of salivary cortisol as well as oxytocin in mothers who are pumping breastmilk in the NICU The third aim is to evaluate the associations between mindfulness intervention and severity of depression as well as anxiety in mothers who are pumping breastmilk in the NICU.

About this study

The purpose of this study is to evaluate the safety and effectiveness of Diazepam in the treatment of high-titer GAD65 associated epilepsy. Subjects will be screened at outpatient clinic visit appointments and identified from lists of patients previously evaluated. There will be a 4-week baseline period where patients will track their seizures. The patient will then be treated with diazepam 5 mg twice daily for 1 week and if tolerating will increase to 10 mg twice daily. Treatment will be continued on the trial for 6 months. Efficacy will be looked at after 2 months of treatment, but the study will be extended to look at tolerability through 6 months. Seizure frequency on treatment will be compared to baseline frequency.

The purpose of this study is to analyze the changes in serum thyroid hormone levels in amiodone-induced thyroiditis (AIT) patients after radiofrequency ablation (RFA) therapy on a portion of thyroid parenchyma in order to safely decrease thyroid hormone levels into the normal range with resolution of thyrotoxicosis.

Physical therapists, personal trainers, and strength and conditioning specialists spend substantial time teaching clients how to strengthen their core. Core muscles provide a stable base for limb movement. About this study

The purose of this study is to evaluate the effectiveness, safety, and tolerability of Nefecon 16 mg per day in the treatment of patients with primary IgAN (Immunoglobulin A nephropathy) at risk of progressing to end-stage renal disease (ESRD), despite maximum tolerated treatment with renin-angiotensin system (RAS) blockade using angiotensin converting enzyme inhibitors (ACEIs) or angiotensin II type I receptor blockers (ARBs).

About this study

Membranous glomerulopathy (MN) is still the most common glomerular disease associated with nephrotic proteinuria (NS). Up to 40% of patients reach end stage renal failure (ESRD), making MN the 2nd or 3rd most common cause of ESRD caused by a primary glomerulopathy. Current treatment options include corticosteroids, alkylating agents, and cyclosporin, but their use is controversial and the associated adverse effects and high cost temper their usage. Experimental data in MN suggests that B cell activation results in immunoglobulin deposition along the glomerular basement membrane causing injury to the membrane and subsequent proteinuria. Drugs that non-selectively inhibit B cells and, these pathogenic antibodies, are closely associated with improved outcomes. Based on the rationale that selective depletion of B cells in humans would prevent the production of nephrotoxic immunoglobulins and subsequent renal injury we recently treated 15 patients with MN with rituximab 1g i.v. twice (day 1 and day 15). Baseline proteinuria of 13.0±5.5g/24h decreased to 9.1±7g, 9.7±8g and 6.5±6 g/24h at 3, 6, and 9 months, respectively (mean ± SD). Analysis of the pharmacokinetic data obtained from this study, however, suggests that in heavily nephrotic patients, rituximab dosed in this fashion results in patients being under-treated. The present study propose to test the hypothesis that rituximab, given in accordance to the standard lymphoma protocol (375mg/m2 x 4), will result in a more effective and profound depletion of B cells, a more complete suppression of pathogenic antibodies, and a higher remission rate of the NS while maintaining a favorable safety profile.
