Requesting An Appointment

If you are interested in being seen by individualized medicine specialists at Mayo Clinic, you or your health care provider can request an evaluation. Each request is evaluated on a case-by-case basis.

You must be able to travel to one of the three Mayo Clinic locations in Rochester, Minnesota; Jacksonville, Florida; or Phoenix/Scottsdale, Arizona. Records from other health care providers cannot be reviewed without an appointment.
