Requesting An Appointment
If you are interested in being seen for individualized medicine services at Mayo Clinic, you or your health care provider may request an evaluation. Each request is evaluated on a case-by-case basis.
You must be able to travel to one of the three Mayo Clinic locations in Phoenix/Scottsdale, Arizona; Jacksonville, Florida; or Rochester, Minnesota. You may also be seen at a Mayo Clinic Health System location if a specialist is available.
The first step is for you or your physician to request an appointment. Mayo Clinic cannot review your symptoms and health records until your appointment is scheduled.
