The purpose of this study is to characterize the safety, tolerability, pharmacokinetics, pharmacodynamics, and antitumor activity of CFT7455 administered orally in subjects with Relapsed/Refractory (r/r) Non-Hodgkin's Lymphoma (NHL) or Multiple Myeloma (MM) administered once a day (QD) as a single agent and in combination with dexamethasone.

The electronic Medical Records and GEnomics (eMERGE) Network Genomic Risk Assessment Study is a multisite study funded by the National Human Genome Research Institute (NHGRI) to investigate the clinical utility of polygenic risk scores for common conditions in adults and children. eMERGE researchers aim to learn about how people’s family history and genetic makeup affect their risk of developing common conditions such as heart disease, diabetes, and cancer, and about how to use this information to help them reduce their risk. The study will recruit 25,000 participants from around the country, including groups who are underrepresented in biomedical research.

Each participant will provide a biospecimen for genetic testing and fill out surveys about personal and family health history. Participants will receive a Genome Informed Risk Assessment (GIRA) report that includes their estimated risk for common conditions based on genetic testing results, clinical variables, and family history. The GIRA report will be uploaded to the participant's electronic health record and is meant to help participants and providers interpret risk for common conditions. A subset of participants will receive their results from a member of the study team in-person or by a virtual appointment.

The purpose of this study is to evaluate the effect of cemdisiran on proteinuria in adults with immunoglobulin A nephropathy (IgAN), who excrete >1 gram (gm) of protein per day despite standard of care, which includes treatment with angiotensin-converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs). These patients are at high risk for progression of kidney disease, which can result in end-stage renal failure.

The purpose of this study is to better understand the link between insulin resistance and dementia. Earlier studies have shown that people with insulin resistance are two to four times more likely to develop Alzheimer’s disease and other dementias. Other studies have shown a link between the drug Metformin (the most widely used medication to treat prediabetes and type 2 diabetes) and a reduction in the development of dementia.

The aim of this study is to investigate if 40 weeks of Metformin treatment (in people 60+ years) enhances brain function and how brain function interacts with the body’s metabolism in people with insulin resistance and in people without insulin resistance.

Insulin is a hormone produced by the pancreas that deals with sugar (glucose) in the blood. When we eat food, blood sugar levels increase, and the pancreas produces more insulin to keep blood sugar levels stable. If blood sugar decreases, then insulin levels decrease. If the body becomes resistant to insulin, then the body requires more insulin to balance out blood sugar levels. If this continues, eventually the pancreas is not able to produce the amount of insulin needed to balance out blood sugar leading to an elevated fasting blood sugar (fasting blood glucose value greater than 100 mg/dL). Insulin resistance is a primary risk factor for developing type 2 diabetes, heart disease, and dementia.

This study is recruiting two groups of people: people with insulin resistance and people without insulin resistance. To qualify for the insulin resistance group, a person would need to have an elevated fasting blood sugar test between 100mg/dL and 140mg/dL. To qualify for the non-insulin resistant group a person would need a blood sugar under 95mg/dL.

In people with insulin resistance the study will be measuring changes in brain functions, cognitive ability, and body’s metabolism. These series of tests are performed twice, once before and once after 40 weeks of taking Metformin. Brain functions are measured through a series of brain scans. Cognitive ability is measured using computerized tests. To measure the body’s metabolism, we measure insulin resistance during an overnight stay, blood tests to measure biomarkers, capacity for exercise, body composition scans, and muscle will be assessed from needle muscle biopsies.

In people without insulin resistance, the study will be collecting baseline measurements on brain function, cognitive ability, and body’s metabolism. These tests are performed once. Brain functions are measured through a series of brain scans. Cognitive ability is measured using computerized tests. To measure the body’s metabolism, we measure insulin resistance during an overnight stay, blood tests to measure biomarkers, and a body composition scan.

The purpose of this study is to determine the feasibility of conducting a randomized clinical trial to assess whether an enhanced virtual world-based cardiac rehabilitation (VWCR) program will provide equivalent participation, adherence, and cardiovascular health outcomes to a traditional center-based, in-person cardiac rehab (CR) program. We hypothesize that implementing our patient-centric VWCR program will be feasible at all 3 participating sites and will offer a sufficient alternative to traditional center-based CR (CBCR).

Researchers are trying to understand how chronic inflammation affects muscle function and responses to exercise.They are also trying to determine if suppressing chronic inflammation using omega-3 fatty acids (n3-PUFA) restores skeletal muscle function and exercise responsiveness in older adults.

The purpose of this study is to evaluate the effect of fenofibrate compared with placebo for prevention of diabetic retinopathy (DR) worsening or center-involved diabetic macular edema (CI-DME) with vision loss through 4 years of follow-up in participants with mild to moderately severe non-proliferative DR (NPDR) and no CI-DME at baseline.

The purpose of this study is to identify the recommended Phase 2 dose (RP2D) of LY3484356 administered as monotherapy and in combination with other anticancer therapies in patients with locally advanced or metastatic ER+ breast cancer or ER+ recurrent, persistent, or metastatic endometrial endometrioid cancer (EEC).

The aim of this study is to develop, in close collaboration with stakeholders, an evidence-based decision aid to inform discussions regarding the treatment of obesity (Obesity Choice).

Though nicotine is known to be a mild analgesic, its use in the peri-operative period to reduce postoperative opioid requirements has only recently been investigated. In nonsmokers it has been shown to reduce postoperative pain and opioid requirements in both female patients undergoing uterine surgery and male patients undergoing prostatectomy. We have recently confirmed these findings in women undergoing uterine surgery by open, laparoscopic and vaginal approaches. In this study we found a 30% mean reduction of 24 hour postoperative opioid requirements following the administration of a 3 mg dose of nicotine nasal spray. Two previous studies with perioperative transdermal nicotine found that increasing doses of nicotine and increasing serum levels of nicotine were associated with higher rates of nausea. In our study, we found that the administration of nasal nicotine was associated with a transient increase of postoperative nausea and vomiting (PONV), but this was limited to the recovery room (PACU) stay. Two studies of nasal nicotine where antiemetics were prophylactically administered did not report the incidence of nausea. In our study, antiemetics were used as rescue medications only. Thus, it is unclear if the prophylactic administration of anti-emetics would effectively counteract the transient emetogenic effect of a small dose of nasal nicotine.

Laparoscopic bariatric surgery is becoming an increasingly popular treatment of morbid obesity. We recently performed a detailed analysis of postoperative pain and opioid requirements among patients that had this type of surgery and found that among non-smokers severe pain (pain >7/10 on a 10 point scale) was commonly experienced (35%) despite substantial amount of postoperative opioid use (mean 68 mg oral morphine equivalents) and non-opioid analgesics in the first 24 hours. Because obesity related sleep disorders exceed 70% of patients undergoing bariatric surgery, and those disorders are exacerbated by opioids, potential therapies that could reduce postoperative opioid requirements in these patients have substantial value. Further, delayed nausea often poses a problem for these patients meeting hospital discharge criteria, and opioids used for analgesia aggravates this problem. Thus, the use of nasal nicotine could reduce the amount of opioid these patients require for postoperative analgesia. Combined with aggressive antiemetic prophylaxis against PONV, nasal nicotine could paradoxically and indirectly reduce the incidence of nausea in these patients by decreasing postoperative opioid requirements. The proposed study is a randomized clinical trial examining how nicotine affects recovery of female non-smokers undergoing bariatric surgery from general anesthesia. We will test the hypothesis that nicotine, administered in a nasal spray immediately before emergence from anesthesia, will reduce postoperative opioid requirement and postoperative pain. A secondary outcome is to assess if the routine administration of prophylactic antiemetic therapy prevents increased PONV associated with nicotine administration.
