A Study Of Self Measured Blood Pressure

About this study

The purpose of this study is to evaluate the effectiveness of self-measured blood pressure (SMBP) on hypertension control. We will utilize provider directed management based on SMBP entered into the electronic health record (EHR).

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Uncontrolled HTN (>130/80) in patient with HTN diagnosis or New HTN diagnosis.

  • Rochester Family Medicine empaneled patient

  • Patient 18 years of age or older.

Exclusion Criteria:

  • BP > 180/120 or HTN emergency at time of potential enrollment.

  • HTN managed by Nephrology.

  • Resistant HTN (Uncontrolled HTN on max dose of ACE/ARB, diuretic, calcium channel blocker)

  • Atrial Fibrillation.

  • CKD4 or higher.

  • Stroke/TIA, Major Adverse Cardiac Event (MACE) within 90 days.

  • Active Cancer Treatment (Chemotherapy or Radiation)

  • Dementia diagnosis unable to check home BP.

  • No access to online patient portal.

  • Significant electrolyte abnormalities at time of enrollment (K+ <3.0 or >5.2; Sodium <130 or >146)

  • Pregnant

  • Terminal illness
