A Study Of Identifying And Addressing Barriers To Durable Home Discharge Among Hospitalized Patients

About this study

The purpose of this research project is to apply user-centered design principles and digital enablement strategies to iteratively evaluate and refine pragmatic clinical approaches to integrate the identification and addressing of barriers to home discharge in routine hospital care.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Admission/Readmission to a medical hospital service.

Exclusion Criteria:

  • Current admission to ICU.

  • Under 18 years of age.

  • Interpreter requirement.

  • Acute intoxication.

  • Aggressive/combative behavior.

  • Sedation

  • Inability to complete the study activities.
