SUMMARY

Brad B. Smith, M.D., specializes in the perioperative management of patients undergoing cardiac and major thoracic surgery, with particular focus on bleeding, thrombosis, hypersensitivity reactions and complex cardiovascular comorbidities. His research examines how abnormalities in coagulation and platelet function, as well as anticoagulant therapy, influence surgical risk and postoperative outcomes. Through retrospective cohort analyses, clinical trials and multidisciplinary collaboration, he evaluates strategies to expand transfusion criteria, improve perioperative hemostasis, and optimize care for patients requiring cardiopulmonary bypass or advanced cardiovascular interventions.

Dr. Smith's work centers on improving perioperative care for patients with increased risk and complexity, an area that often presents challenges for clinicians. His research addresses rare syndromes, cardiac transplant outcomes, perioperative drug reactions and the care of patients with coronary stents undergoing surgery for other conditions. He integrates clinical epidemiology, anesthesiology practice data and multidisciplinary collaboration to clarify perioperative complications and advance evidence‑based strategies for safer surgical care and improved outcomes.

Focus areas

  • Perioperative bleeding and transfusion risk. Dr. Smith examines the mechanisms and management of coagulopathy in cardiac surgery, with particular emphasis on cardiopulmonary bypass. His work evaluates coagulation testing, anticoagulant management and transfusion strategies to reduce bleeding complications. The goal is to optimize platelet and coagulation function and improve perioperative outcomes in surgical populations at increased risk.
  • Complex cardiac surgical populations. Dr. Smith studies perioperative outcomes in patients undergoing cardiac, thoracic and vascular procedures who have rare syndromes or complex medical conditions. His research includes investigations of transplant outcomes, surgical cancellations and perioperative risk factors to better define clinical pathways for patients with complex medical conditions.
  • Perioperative hypersensitivity reactions. Dr. Smith explores the causes, diagnosis, and prevention of allergic reactions and anaphylaxis that may occur around the time of surgery. This includes drug-related hypersensitivity, reactions to medical devices, and multidisciplinary evaluation strategies to improve the recognition and management of these rare but potentially life‑threatening events.
  • Coronary stents and surgical risk. Dr. Smith evaluates outcomes for patients with coronary artery stents who require other types of surgery. He assesses optimal timing of procedures, management of antiplatelet therapy and associations with major adverse cardiac events to inform safer perioperative decision-making.
  • Patient-centered perioperative outcomes. Through large-cohort analyses and clinical outcome metrics, Dr. Smith examines postoperative pain management, hospital utilization and recovery following major surgical procedures. This work seeks to identify care strategies that enhance patient recovery and reduce complications.

Significance to patient care

Many people who need surgery have underlying heart disease, bleeding risks or medication sensitivities that make surgery more challenging. Dr. Smith studies how these factors affect patients before, during and after surgery, especially those having heart or chest procedures. By finding safer ways to manage blood clotting, medications and allergic reactions, he helps care teams reduce complications such as bleeding, heart and lung issues, and unexpected drug reactions. This supports better decision-making throughout the surgical process, leading to safer care, fewer interventions and improved recovery.
