A Mixed Methods Study Of AKI Survivor Communication Guides

The purpose of this study is to pilot test a newly developed communication tool designed to enhance information transfer between health care providers caring for survivors of acute kidney injury (AKI), evaluating its feasibility, acceptability, and preliminary impact on care coordination and clinical workflow efficiency. We hypothesize that the new communication tools will be feasible and acceptable to patients, caregivers, and clinicians alike.
