Carrie Nelson, M.D., is medical director for outpatient care management and APP Advisors with Advocate Physician Partners, an organization of more than 4,000 clinically integrated physicians including more than 1,100 primary care physicians and 2,900 specialists.
Dr. Nelson's primary focus is on refining and supporting the outpatient care management program, developing tactics for success within the accountable care organization, and consulting with other health care systems on Advocate's clinical integration program and accountable care organization strategies.
Dr. Nelson has practiced family medicine for more than 20 years and has been working on issues of health care quality and patient safety since 1999. She has held several volunteer and professional roles for health care improvement in both the hospital and ambulatory environments.
Board-certified in family medicine, Dr. Nelson completed medical school at Rush Medical College in Chicago. She also completed a graduate degree at the University of Wisconsin-Madison in administrative medicine and population health.
