Clinical Research Training Program

Program Description
The aim of the Transplantation Surgery Fellowship Program at Mayo Clinic in Rochester, Minn., is to provide our fellows with the best possible foundation for a career in transplantation surgery.   The American Society of Transplant Surgeons has accredited Mayo Clinic for training in liver, kidney and pancreas transplantation.  Each year, we perform 75-110 liver transplants, more than 200 kidney transplants, and approximately 20 pancreas transplants.  The fellowship program is overseen by the Mayo Graduate School of Medicine under the supervision of the Graduate Education Committee.  Therefore, Fellows at Mayo are considered trainees and comply with all regulations regarding trainees including the 80-hour work week as stipulated by the Mayo Graduate School of Medicine and the Accreditation Council for Graduate Medical Education (ACGME).

The Mayo Clinic transplantation surgical fellowship consists of two different tracks.  One fellow is selected yearly for each track.  The first, designed for individuals who wish to pursue a career in academic transplant surgery, includes a year of laboratory-based research prior to the two clinical years.  The second position consists only of two clinical years.

The research experience includes opportunities for research in endothelial cell biology, immunology, artificial liver support devices, and other basic research projects in all facets of transplantation.  This year can also include training in human pancreas islet transplantation.  Our fellows have been productive in the laboratory year and have presented at both national and international meetings.  In addition, most fellows have completed a clinically oriented manuscript during this year.  We feel strongly that a year of research is an important beginning to a career in transplantation surgery.

Program Structure
During the two years of clinical training, the surgical fellows spend approximately 6 months on the liver transplant service and 6 months on the kidney pancreas service.  The remaining 12 months are spent on the donor rotation.  During these 12 months, the fellows may also choose to perform an elective rotation at another Mayo Clinic Transplant Center (Scottsdale or Jacksonville) or on a hepatobiliary surgery service.

The Liver Transplant Service
The goal of this rotation is to gain significant experience with liver transplantation, including the pre- and post-operative management of adult and pediatric recipients.  In addition, the fellows become proficient in performing multi-organ procurements as the operative surgeon.  The surgical fellows also become familiar with common medical issues in liver transplant patients such as the management of hyponatremia, SBP, intractable ascites, addiction issues, and fulminant hepatic failure.  Procedures such as large volume paracentesis and liver biopsies are also part of the training.  Lastly, the fellows are exposed to innovative treatments, including transplantation, for extrahepatic cholangiocarcinoma.

The Kidney Pancreas Transplant Service
The goal of this rotation is to gain experience in kidney transplantation (living and deceased donors) and pancreas transplantation.  The major emphases of this rotation are surgical experience as well as the post-operative management of these patients and their complications.  The surgical fellows become familiar with the pre-transplant evaluation of potential recipients, operative planning, immunosuppressive management and the medical complications of transplant recipients.  Unique aspects of the Mayo Clinic – Rochester kidney transplant program include positive crossmatch and ABO incompatible kidney transplantation.  In addition, the fellow will gain experience with kidney transplantation for the pediatric recipient.

Donor Service
The aim of this rotation is to provide the fellow experience with multi-organ abdominal procurements and living donor nephrectomy.  The fellow also will gain additional experience with central venous line placement and hemodialysis access procedures during this rotation.

Program Objectives/Expectations
The goal of the fellowship is to train competent transplant surgeons who readily find employment following graduation.  To this end, each fellow is assigned a mentor during the program.  In addition, each fellow is provided with ample opportunity to complete clinical research projects and to present at national meetings.
