Qualifications
To apply, you must have been fully trained during your residency in both head and neck cancer surgery, and facial plastic surgery. 

You must be eligible for licensure in the State of Arizona. 

If you are a foreign medical graduate, you must have recent (within two years of application) ECFMG certification.
