Credit Mayo Foundation is accredited by the Accreditation Council for Continuing Medical Education to sponsor continuing medical education for physicians and adheres to all ACCME standards regarding industry support of continuing medical education.

Registration To register, please print out and complete the registration form and return with payment. If you are using a credit card, you may fax your registration form to 1-602-301-8323. Tuition is $480 for physicians active in practice and $340 for nurse anesthetists. Other discounts are available.
Tuition includes continental breakfasts, refreshment breaks, "lite fare" lunches, Welcome Reception and a course syllabus.
Although it is not Mayo Clinic Scottsdale's policy to limit the number of registrants for a course, conference room facilities may necessitate closing of enrollment. Early registration is strongly recommended.

Recreation Golf: The Pro Shop has a tee-time block reserved on Friday, January 15, starting at 2:30 p.m. A special rate of $45 has been coordinated for 18 holes. To make a reservation, call 1-602-951-5427.
Tennis: The Tennis Club has courts reserved on Friday, January 15, from 2:00-4:00 p.m. The rate is $15 per hour/per court. To reserve a court, or arrange a match of singles or doubles (let them know your level of play), please call the Tennis Club at 1-602-948-7111.
"The Spa": Special invigorating packages have been designed for our group at "The Spa" at Marriott's Camelback Inn Resort which is located just down the street from Marriott's Mountain Shadows Resort & Golf Club. This 27,000-square foot, world-class European health spa features state-of-the-art fitness and exercise facilities, full-service Spa Salon including indulgent body and beauty treatments. Jeep Tour: Camelback Adventures is ready to take you on a four-wheel drive, narrated adventure on Friday, January 15, from 2:00-6:00 p.m. It's a great way to see and experience the Sonoran Desert. Additional information will be sent along with your course confirmation letter.
Other activities available: basketball, volleyball, horseshoe pit, table tennis, shuffleboard, two Marriott 18-hole championship golf courses nearby, hiking, and a shopping trolley to The Borgata, Scottsdale Fashion Square and Old Scottsdale. You may also find time to venture out to the Grand Canyon (four hours by car), Sedona and Red Rock Country (two hours by car), or to tour Scottsdale, a recognized center for the arts.
Mexican FiestaMarriott's Mountain Shadows' authentic "South of the Border Town" will come to life for a "Mexican Fiesta" on Thursday evening, January 14, from 6:00-8:00 p.m.
Please plan to join us at this spectacular buffet dinner which will feature entertainment by a mariachi band and mexican artisans.
Tickets are $20 for adults and $10 for children under 12. Pre-registration is required.
