Federalwide Assurance (FWA)

The Mayo Clinic Institutional Review Board has a Federalwide Assurance (FWA) on file with Office for Human Research Protections (OHRP). This assurance agreement is between Mayo Clinic and the OHRP, which acts on behalf of the secretary of the Department of Health and Human Services.

The Federalwide Assurance outlines the principles and guidelines that govern Mayo Clinic and its faculty and staff in the protection of the rights and welfare of participants in research conducted at or sponsored by Mayo Clinic.

The Federalwide Assurance number for Mayo Clinic is FWA00005001 and is valid through Nov. 30, 2017. The Department of Defense Addendum number is F50420 and is valid through Jan. 21, 2014.
