Communities Disproportionately Affected by Health Inequities

About this study

The proposed study will determine if a multi-level CHW led SDoH intervention can improve general well-being (GWB). The study will be carried out in six communities targeting populations most disadvantaged by health inequities. Informed by community partners, each site has selected the SDoH(s) they will target. Each site will conduct CHW led community specific multi-level interventions consisting of individualized navigation to SDoH resources and services.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria: 

  • Individuals living within the target community who do not plan to move out of the community within the next six months (all sites)
  • Having two un-met SDOH needs (all sites)
  • University of Miami/ Healthy Little Havana Inclusion Criteria
    • Age 18 and over
  • Health Choice Network/ Broward Community Health Center/ Genesis Community Health Center Inclusion Criteria
    • Age 18 and over
  • Florida International University Inclusion Criteria
    • Age 50 and over
    • Individuals who identify as LGBT
  • Mayo Clinic Florida Inclusion Criteria
    • Age 21 and over
    • Black/African American; Hispanic or Non-Hispanic ethnicity
  • Florida A&M University/ Mother Care Network Inclusion Criteria
    • Women between the ages 13-19 years o Black/African American; Hispanic or Non-Hispanic ethnicity
  • University of Florida Inclusion Criteria
    • Women aged 18 and over
    • Pregnant and recently post-partum minority women
    • Black/African American or Caucasian race; Hispanic or Non-Hispanic ethnicity
    • Are uninsured or underinsured

Exclusion Criteria: 

  • Are unable or unwilling to consent
  • Plans to move out of the community during the next six months
  • Enrollment in a research study addressing SDoH in the last three years.
  • Prisoners
