Positions
One candidate will be selected for appointment this year for the PGY2 Critical Care Pharmacy Residency at Mayo Clinic in Florida.
The program begins around June 29.

How to Apply
Your application and all supporting documentation must be received by January 10 of the year in which you would enter the program.

Step 1: Mail an official transcript from your college of pharmacy and a curriculum vitae to:

Step 2: Complete the online Mayo School of Health Sciences application form, noting the following instructions:

A. Under Education Information:

  • Do NOT include information about your high school or pre-pharmacy training
  • Do include information about your pharmacy school
  • Do include information about your PGY1 pharmacy residency

B. Do not complete the section titled Standardized Testing.

C. Under Recommendations, enter the information needed to provide three letters of recommendation. Recommenders who wish to submit letters of reference on paper should download and print a copy of the reference form and follow the directions for paper references. References on paper should be mailed to the Program Director.

Step 3: Applicants considered for an appointment will be invited to visit Mayo Clinic for a personal interview in Florida with the program director and select faculty. An interview can be arranged only when your application file is complete.
