The Mayo School of Health Sciences Physician Assistant (PA) Fellowship in Hospital Internal Medicine (HIM) prepares individuals for professional practice in hospital medicine. The HIM PA Fellowship curriculum is based upon the Society of Hospital Medicine's "Core Competencies." These include knowledge, skills and understanding of health care areas which are:
1. By the end of the fellowship year, the PA fellow will be able to exhibit a working knowledge of the following disease entities:
- Acute coronary syndrome
- Acute renal failure
- Alcohol and drug withdrawal
- Asthma
- Cardiac arrhythmia
- Cellulitis
- Chronic obstructive pulmonary disease
- Community acquired pneumonia
- Congestive heart failure
- Delirium and dementia
- Diabetes mellitus
- Gastrointestinal bleed
- Hospital-acquired pneumonia
- Pain management
- Perioperative medicine
- Stroke
- Urinary tract infection
- Venous thromboembolism
2. By the end of the fellowship year, the PA fellow will have experience performing the following skills/procedures:
- Arthrocentesis
- Chest radiograph interpretation
- Electrocardiogram interpretation
- Emergency procedures
- Lumbar puncture
- Paracentesis
- Thoracentesis
- Vascular access
- Problem-focused physical exam
- Focused patient presentation to supervising faculty
3. By the end of the fellowship year, the PA fellow will have a working knowledge and understanding in the following health care areas:
- Care of the elderly patient
- Care of the vulnerable populations
- Communication
- Diagnostic decision making
- Drug safety, pharmacoeconomics, and pharmacoepidemiology
- Equitable allocation of resources
- Evidence-based medicine
- Hospitalist PA as a consultant
- Hospitalist as a teacher
- Information management
- Leadership
- Management practices
- Nutrition and the hospitalized patient
- Palliative care
Rotations
The HIM PA Fellowship is comprised of 12 rotations in the various specialty areas:
- Hematology/Medical Oncology (elective)
- Palliative Medicine (required)
- Cardiology (required)
- Emergency Medicine (elective)
- Physical Medicine and Rehabilitation (required)
- Transplant Medicine (elective)
- Interventional Radiology (required)
- Endocrinology and Nutrition (required)
- Hospital Internal Medicine consult service (required)
- Hospital Internal Medicine (night service) (required)
- General Hospital Internal Medicine (required)
Subspecialty rotations may vary from two to four weeks in length.
Grading or Evaluation
Mayo School of Health Sciences uses evaluative tools that include:
- Written examination
- Demonstration of skills
- Self-assessment exercises
- Faculty reviews
Specifically the HIM PA fellow will be evaluated by the following assessment tools:
- End-of-rotation evaluations
- End-year manuscript submitted to peer-reviewed journal for publication
- Midyear and end-year formal comprehensive evaluations
- Competency checklist components completed by end offellowship training
- End project for core competency "modules" with various assessment tools
- End-year Summative Portfolio
