Each year, Mayo School of Health Sciences accepts up to 30 students in its Masters of Nurse Anesthesia Program at Mayo Clinic in Rochester on a competitive basis.

Classes begin on the Monday following Labor Day each September. The traditional program is completed 30 months later, in mid-March.

How to Apply
Please note: The Master of Nurse Anesthesia application is in the process of being updated.  While you may access and review the current application form, final changes to the form will appear on the application after August 1.  You will be required to use the updated form for  submission.

Your application and all supporting documentation must be postmarked no later than October 1st (send via trackable shipping ie: Fed Ex, UPS etc.) for admission to the classes beginning in September of the following year. Please complete the following steps:

Traditional Master of Nurse Anesthesia Program Application Process
Step 1: To apply to the Traditional Master of Nurse Anesthesia Program, you must submit the following materials:

  • Application Checklist & Supplemental Forms
  • Completed online Application for Admission. After you create an account and/or log-in, please select Application for Admission.
  • Personal statement (600 or fewer words).
  • Official transcripts of all college and/or university credits (sealed in the school’s envelope).
  • Three complete references (in unopened signed envelopes) including:
    1. An academic reference
    2. A work-related reference from a physician who knows your capabilities (not a resident or fellow)
    3.A current nurse-manager reference
  • Apply Yourself references not acceptable; narrative references required!
  • Copies of all current nursing licenses and verification of status in all states in which you are licensed (Can be obtained from each state's Board of Nursing Web site.)
  • Copy of bachelor's degree.
  • Additional supporting information, such as advanced nursing specialty certification, awards, publications and honors.
  • A nonrefundable $50 application fee with credit card through the Apply Yourself Web site.
  • A copy of your GRE scores (with official copy sent directly to the program office) Mayo's GRE code: 5784.

Step 2: Applicants selected as preliminary candidates will be notified by the first week in November. If you are considered for an appointment, you will be asked to visit Mayo Clinic in Rochester for a personal behavioral interview with the program director and selected faculty. Interviews are held on specific days during November and December. Final candidates for the program are selected after all interviews are completed.

Step 3: Appointments are made by Mayo School of Health Sciences. Appointment letters are mailed by mid-December. Your reply is expected by the end of January. Payment of the $300 slot retention (non-refundable) fee is required upon acceptance into the program.

Nurse Anesthesia Degree Completion Master Program Application Process

To apply to the Nurse Anesthesia Degree Completion Master Program, the following items must be submitted:

For Mayo Clinic alumnus
  • Completed online application form.
  • Personal statement (600 or fewer words,).
  • Transcripts from all post-secondary academic institutions that are not on file.
  • Non-refundable application fee of $50 with credit card through Apply Yourself website or check payable to Mayo Clinic.
  • Unopened reference letters from a work-related physician and nurse manager. Must have signature across sealed envelope flap.
  • GRE scores (with official copy sent directly to the program office).
    Mayo’s GRE code: 5784

For all other CRNA Degree Completion Master Program applicants

  • Completed online application form.
  • Personal statement (600 or fewer words,).
  • Unopened official transcripts from all post-secondary academic institutions attended
  • Non-refundable application fee of $50
  • Unopened reference letters from a work-related physician, a supervisor and an educator.
  • GRE scores (with an official copy sent directly to the program office).
    Mayo's GRE code: 5784.
Background checks must be complete (separate fee will be charged)
