The application requires the following supporting documentation:
- Official transcripts of all colleges and/or university credits.
- A signed Declaration of Intent to Complete Degree and Academic Requirements (Verification Statement for graduates).
- Three letters of recommendation from professional or academic references.
- One page typed (10 point. font or larger) biography, including professional
interests and career goals.
- A nonrefundable $25 application fee.
