Positions
One candidate is accepted each year for the Physical Therapy Geriatric Residency at Mayo Clinic in Arizona on a competitive basis. The program begins in September.
How to apply
Your application and all supporting documentation must be received by April 30, 2013.
Complete the online Application for Admission
After you create an account and/or log-in, please select Application for Admission.
You will submit a non-refundable $35 application fee, payable by credit card via the online application.
- Submit the following materials:
- Transcripts
- Two letters of recommendation (form and instructions)
- Cover letter
- Curriculum vitae (CV)
Please send the required materials that are not completed online, as well as any transcripts not sent directly to MSHS, in one envelope to:

- The selection committee carefully reviews each application. Individuals considered for an appointment may be invited to Mayo Clinic in Arizona for an in-person interview or interviewed by telephone. In some cases, other arrangements may be made.
- Appointment letters are mailed approximately June 1. Your reply is expected within 10 business days.
