Application Process
Positions
Two fellows will be accepted each year for the Cardiology Nurse Practitioner Fellowship Program. Please refer to the application/admission timeline.
How to Apply
Step 1
Complete the online Application for Admission.
After an account is created, log-in and select Application for Admission.
A non-refundable $35 application fee is payable by credit card via the online application.
Step 2
Submit the following materials:
- Verified transcripts from baccalaureate nursing and masters/doctorate prepared nurse practitioner (NP) school. If at the time of the application process, transcripts are pending graduation, initial unofficial copies may be submitted
- Copy of licensure or board eligibility confirmation letter as a NP in state of AZ
- Three letters of recommendation, sent directly to MSHS, from clinical preceptors or mentors
- Statement of intent (max. 300 words) describing how the fellowship will continue their goals in a cardiovascular field.
Please send all required materials which are NOT completed with the online application, plus any transcripts not sent directly to MSHS, in one envelope to:
