Positions
There is one position available in the Adult Reconstructive Surgery Fellowship each year on a competitive basis.

How to Apply
To apply for the Mayo Clinic Arizona Adult Reconstructive fellowship, please follow the process below:

E-mail a completed scanned copy of your application and supporting documentation to boord.beth@mayo.edu or mail your application to the following address: Mayo Clinic Arizona, 5777 East Mayo Blvd, Phoenix, AZ 85054 Attn: Beth Boord, Adult Reconstructive Fellowship Coordinator. Applications are accepted between September 1st and December 31st each year. Interviews are conducted between January and March the following year and the match takes place in May.

In addition to the standard application you will need to submit the following additional supporting materials with your application:

  • Curriculum vitae. Describe your activities with professional societies, publications, and any other relevant information about your education or experience.
  • Copy of graduate school and or medical/dental school diploma.
  • Official final transcripts showing degree earned and date conferred from each graduate school, dental school, and/or medical school you attended. Provide certified or notarized English translations of documents written in a language other than English.
  • Official test transcripts for all applicable examinations (USMLE, LMCC, COMLEX, NBOME, FMGEMS, FLEX, or NBME).
  • Valid ECFMG certificate. Required if you graduated from a medical school outside the United States or Canada.
  • Three letters of recommendation, from physicians or faculty members who are thoroughly familiar with your work. Each letter should include the writer’s opinion of your professional, academic, and personal qualifications, as well as his/her opinion of your potential success in the field for which you are applying. Letters of Recommendation should be included with your application or if you have waived the right to view your letter then please have the letters faxed to 480-342-2027 or e-mailed to boord.beth@mayo.edu.
  • Copy of completion certificate from each of your prior residency and/or fellowship training programs.
