Family Medicine -- General - Rochester

Description:

The Family Medicine elective is a subinternship that consists of two components and is designed to involve the fourth year medical student as a G-1 Resident in the Family Medicine Department. The first possible component consists of the Family Medicine inpatient service at St. Marys Hospital. Three to four family medicine residents and a consultant, as well as a senior Family Medicine resident staff this active hospital service. The medical student will assume primary responsibility for the patients that he or she admits to the Family Medicine Service. The second component of the fourth year elective consists of an ambulatory care experiences at the Kasson Mayo Family Practice Clinic. Students will be assigned their own schedule of patients on a daily basis and will be precepted by Family Medicine consultants available at the clinic.

Specific Goals:

  1. Gain an understanding of family medicine in the inpatient and outpatient ambulatory care arena.
  2. Observe how the practice of family medicine is integrated into the spectrum of care delivered by a large multi-specialty institution.
  3. Evaluate family medicine as a possible career choice.
  4. Gain background information about the selection of a family medicine residency.
  5. Develop definitive care plans in an outpatient and an inpatient setting.
  6. Refine history and physical, as well as assessment and management skills.
  7. Obtain introduction into practice management skills, in an outpatient and inpatient setting, with respect to clinic management, billing, coding, etc.

Activity Outline:

  1. Attend daily rounds with the resident group and family medicine consultant.
  2. Complete comprehensive history and physical examinations on patients admitted.
  3. Formulate comprehensive assessments, differential diagnosis, and treatment plans for each patient admitted to the hospital service.
  4. Present patients each morning on rounds, as well as follow the patient through the entire course of care.
  5. Attend all applicable conferences and resident seminars while on inpatient or outpatient services.
  6. In the ambulatory care setting at the Kasson Mayo Family Practice Clinic, see and evaluate each patient assigned to the student on his or her "clinic" day. Perform appropriately focused histories and examinations, and formulate possible diagnosis, assessments, and plans. Present each case to the preceptor present for the day and finalize diagnostic and treatment regimen.
  7. Discuss coding and documentation procedures with the preceptor on each patient seen during the day.

Method of Evaluation:

Students are evaluated by the faculty and residents as a team, both in the hospital and in the ambulatory care setting.
