The Primary Care Sports Medicine Fellowship is based out of the Department of Family Medicine at Mayo Clinic in Jacksonville, Fla. It is a one-year fellowship that accepts one fellow each year.

The Primary Care Sports Medicine Fellowship was founded in 2002 by a collaborative effort of dedicated physicians at Mayo Clinic in Florida. It accepted its first fellow that same year and has graduated one fellow a year without interruption since that time.
The Primary Care Sports Medicine Fellowship is located at Mayo Clinic in Florida, which is fewer than 3 miles from the Atlantic Ocean.
