Licensure

Allopathic

Registration with the Arizona Medical Board is required before beginning your residency program.

  • This process requires two to three months to complete, so please complete the Application for Registration as an Unlicensed Allopathic Physician form in MedHub as soon as possible. You may disregard the payment portion of the application, as Mayo Clinic will cover the cost of licensure.
  • Complete the Citizenship Statement (evidence of ID is included with the Citizenship Statement).
  • Trainees eligible for full licensure should access the Arizona Medical Board.

Note: If any area of the application is left blank (except the payment portion), it will be returned and will delay processing your application as well as your Mayo appointment.

Osteopathic

Registration with the Arizona Board of Osteopathic Examiners is required before beginning your residency program.

  • This process requires two to three months to complete, so please complete the Application for Registration as an Unlicensed Osteopathic Physician form in MedHub as soon as possible. You may disregard the payment portion of the application as Mayo Clinic will cover the cost of licensure.
  • Complete the Citizenship Statement (evidence of ID is included with the Citizenship Statement).
  • Trainees eligible for full licensure should access the Arizona Board of Osteopathic Examiners.
