Virtual Attendance through Second Life
Registration for this activity via Second Life® must be completed by Feb. 15, 2012. This is not the registration site for the "live onsite activity" being held concurrently at the Princess Resort in Scottsdale, Ariz.
The registration fee provides the registrant with flexible attendance. Attend as much or as little of the activity as your schedule allows.
The Record of Attendance will be provided to the individual who registered for the activity.
When registering online, please be sure to "check-out" after your profile is completed/updated.
To take advantage of the orientation session(s), please have your Avatar created by Monday, February 13, 2012. Orientation session(s) are noted in the Additional Information tab to the left.
A letter of confirmation will be sent upon receipt of payment and completed registration form.

No refund will be granted unless a written notice of cancellation is received. If registration must be canceled, tuition less a $75 administrative fee will be refunded when notification is received on or before Monday, Feb. 6, 2012. No refunds will be made after this date. Technical issues relating to non-Mayo equipment is not a basis for a refund request.
A Record of Attendance (ROA) is provided to the individual who registered for the activity. The Record of Attendance allows attendees to calculate their own credits of participation in the educational activity. At conclusion of course, an email message will be sent to each registrant where you will enter number of hours attended each day. For all those who submit responses to the ROA survey link, a certificate will be generated and sent.
The Record of Attendance can be used for requesting credits in accordance with state licensing boards, specialty societies, or other professional associations.
