The theme for the 2014 Minnesota Geriatric Care Conference is transitions that many elderly confront during this period of their lives. This population often moves from their homes to living in a care facility, transitioning from independence to care. Other transitions might be from wellness to chronic illness, from acute care to long term care, from abundance to loss. Their opportunities at this stage of their lives might be to become and stay active, and achieve purpose in their lives. Challenges often faced by the elderly and their families are decisions about level of care and reaching those decisions in a manner that will enhance their family relationships. Additionally, the conference will address some of the more common chronic conditions that present potential difficulties for care.
