Online registration is preferred, but a paper form to mail or fax can be downloaded from this site. The registration fee is $850 for physicians and $640 for retirees, trainees and NP/PAs. This fee includes tuition, comprehensive electronic syllabus, breakfasts, break refreshments, lunch on Friday, Welcome Reception and the Gala Dinner.
