Registration
Register online or at www.mayo.edu/cme. The registration fee includes tuition, continental breakfast, light lunch and break refreshments. Although it is not Mayo School of Continuous Professional Development policy to limit the number of registrants for a course, the meeting space may dictate closure of enrollment. Registrations are accepted on a first-come,
first-served basis. A letter of confirmation will be sent upon receipt of payment and completed registration form. Please present this letter when checking in at the meeting registration desk.
Cancellation Policy
Your registration fee, less a $20 administrative fee, will be refunded when the Mayo School of CPD receives written notification on or before February 5, 2010(fax#: 904-953-2954). No refunds will be made after February 5, 2010.
